Root cause analysis is a systematic approach used to identify problems within an event and create a plan to prevent the problem from recurring in the future. To be effective, a timeline of events is created to help identify those areas that may be the reason for the problem or event, and the relationship between the causal factors and those factors identified as the reason for the event to occur. In the case of Mr B, an investigation into the events leading to his untimely death would be necessary. Once the problem is identified and described, event data is collected and formatted into a timeline. From the events, any problems in patient care that may have contributed to the final outcome are identified and determined whether they are causal. Appendix A outlines the chronology of the event. After examining the timeline of events surrounding Mr. B, there are several causal factors that led to this sentinel event. These include inadequate staffing, failure to identify worsening trends, failure to follow conscious sedation policy, inadequate observation and monitoring, failure to respond to alarms, inadequate assessment of home medications, dosage of medications, timing of appropriate medication administration, and failure to initiate cardiopulmonary resuscitation. in a timely manner.
