National Health Insurance (NHI) was proposed by the African National Congress (ANC) in 2007, but until now proposals like these have not been taken seriously as South Africa has tried to achieve a health insurance system that provides accessible health care to all. The main intent of this NHI reform is to increase up to 5% of gross domestic product (GDP) in additional taxes to reach a total government expenditure equivalent to 8% of GDP. The ANC has to date procured 4 documents relating to the NHI and some proposals contain important financial and efficiency implications which are unclear. The NHI has made many recommendations that will be discussed further. The NHI wants to establish an “NHI Fund” which will raise revenue, pool revenue and purchase all health services in South Africa. This fund will be managed by a chief executive who will be appointed by the Minister of Health “similar to the way in which the head of SARS (South African Revenue Service) reports to the Minister of Finance”4. The NHI Fund wants to operate as a separate body from the South African Civil Service as this is practiced internationally and international experience has demonstrated its benefits. The NHI proposes that the National Department of Health supervise and “remain an important service provider through its facilities and facilities at the national, provincial and district levels.” The NHI wants all existing public health system purchases to be made by the NHI. They also want a district team to be created to provide health services to the population. They propose that there is one team for every 10,000 people.
